IMMUNIZATIONS & ADMITTANCE

IMMUNIZATIONS: In order to attend Camp Fowler your child must have a current IMMUNIZATION list on file with us as required by the NEW YORK STATE DEPARTMENT OF HEALTH.

NEW FOR 2017: If your child is currently enrolled in a NY Public School, Fowler has access to her/his immunization records. If your child is home schooled or attends a school outside of New York State, please read the following:

HOW DO I OBTAIN AN IMMUNIZATION LIST FOR MY CHILD : Call your school nurse or family doctor for a copy of their records.

MY CHILD IS A RETURNING CAMPERDO I NEED TO BRING A NEW COPY EACH YEAR?  NO - you do not need to bring a list each year UNLESS: the child has new inoculations or your record in the Fowler office from previous years is not found.

AS A RETURNING CAMPER, DO I NEED TO CONTACT THE FOWLER OFFICE TO CONFIRM MY CHILD'S RECORDS?    ⁃    NO. As registrations are processed Fowler staff reviews in-house records. If we do not find your child's records we will contact you.  We strongly advise that you keep current immunization lists in your files at home that you may copy should it become necessary to do so.

DOES MY CHILD NEED SPECIFIC IMMUNIZATIONS TO ATTEND CAMP?  NO. We do not require specific immunizations. We do recommend that you child has a current tetanus immunization.  NYS Dept or Health requires a list of what your child has been inoculated for but does not require specific immunizations.

CAN MY CHILD COME TO CAMP WITHOUT IMMUNIZATIONS?   YES. If your child has not been inoculated for religious, personal or medical reasons he/she may still come to camp. A letter from the doctor, religious officiant or yourself may be substituted for an immunization list and will be filed in our records as such.  We are monitored by the NYS Department of Health and our records are periodically examined for compliance. An official letter as stated herein complies NYSDOH regulation for IMMUNIZATION records on file.

SHOULD I FAX, MAIL OR EMAIL MY CHILD'S RECORDS TO CAMP? NONE OF THE ABOVE!!!  We no longer accept faxes, mailed copies, or emailed copies of any medical records.

IT IS REQUIRED THAT YOU BRING ALL YOUR MEDICAL DATA TO CAMPER CHECK IN.

FAQs: MEDICATION AUTHORIZATION

REGARDING MEDICATIONS, PRESCRIPTIONS and NON-PRESCRIPTION DRUGS & OUR NEW MEDICATION  FORM

Camp Fowler is subject to governing rules from the New York State Department of Health (NYSDOH) regarding medications and dispensation to children while at a summer camp.

As defined by the NYSDOH a "MEDICATION" is ANY substance taken to maintain and/or improve health. This includes over-the-counter drugs, prescription drugs, vitamins and natural remedies.

NYSDOH requires a physician's signature and Parent/Guardian signature on an "APPROVED FORM" to authorize any "MEDICATION" (as defined above) dispensed by our camp nurse to your child.

Our required form to approve medication is the "MEDICATION Form". This form is required completed to attend camp. How it is completed is determined by your child's medication requirements. Read the options below to determine how you will complete the form. 

If your child is not to be dispensed any "MEDICATION" including non prescription medications while at camp your signature/initials (as well as your child's information) is all that is required to complete the form

If you would like your child to receive Fowler provided non-prescription medications (see the form for specific non prescription meds we provide) both you and your child's doctor must authorize the  form with your signatures.

    ⁃    Without both signatures on the form we can not dispense any "medications" to your child.
    ⁃    Please CROSS OUT directly on the form any of Fowler's over the counter meds that your child SHOULD NOT HAVE.
    ⁃    Unless otherwise instructed by your child's doctor Fowler's over the counter meds are dispensed according to the manufacturers instructions on the package
 
If your child requires non-prescription medications which are NOT PROVIDED by Camp  then the following steps must be followed for the nurse to be able to dispense non-prescription medications you bring to camp.
    ⁃    The medication schedule on the Medication Form (same as the one used for prescription drugs) must be completed for each non-prescription medication you are providing.
    ⁃    Both you and the doctor must authorize dispensation with signatures on the Medication Form.
    ⁃    You must bring the provided non-prescription medicine in its original container at camper check in.
    ⁃    The camp nurse will dispense according to the doctors instructions on the Medical Form.
 
If your child needs to take PRESCRIPTION MEDICATIONS at camp, the following steps must be taken for the camp nurse to dispense:
    ⁃    All prescribed medications must be listed on the Medication Form with the following information described: diagnosis, name of the medication, dosage, frequency and instructions. If the child takes more than 3 medications use the back of the form to provide the remaining required information.
    ⁃    The prescribing doctor must sign the form and provide a daytime phone number.
    ⁃    You the parent/guardian must sign the form.
    ⁃    You must bring the medication in the original bottle with the label attached to camper check in. The label on the bottle along with the doctor's signature on the Medication Form compromises the "prescription". The medication information the doctor provides on the form must match the information provided on the label.


QUESTIONS OFTEN ASKED . . .

DOES MY CHILD NEED A PHYSICAL TO ATTEND CAMP?

NO - Fowler does not require a physical to attend camp. We require a current immunization list,  a properly completed MEDICATION FORM and a parent signed RELEASE & WAIVER FORM. These must be brought to camper check in at arrival and submitted to staff and the Camp Nurse for review.

 

WHAT IF I FORGET TO BRING THE Medication Form TO CAMPER CHECK IN?

Medications can not be dispensed to your child without this form properly completed, signed and on file with the camp. Always make multiple back up copies of all medical forms.

 

DOES MY CHILD NEED AN ACTUAL "SCRIPT" FROM THE DOCTOR?

NO. You can provide one if you wish however we need the Medication form completed and signed by both you and the doctor. The Medication Form  properly completed and an original container of the proscribed medication with the label intact allows the camp nurse to dispense to your child.

THE MEDICATION  FORM MUST BE SUBMITTED AT CAMPER ARRIVAL. Fowler does NOT accept faxes, scans or paper mailed medical forms.

 

FAQs: RELEASE & WAIVER

Release & Waiver Agreement(required with parent signature for camp attendance)

Camp Fowler requires an "assumption of risk" and "risk disclosure" agreement to be reviewed, signed and submitted to camp at check in.

Summer camps are inherently filled with activities of adventure like boating, hiking, swimming and running to name a few. These activities involve risks. As a parent you need to be aware of these risks and that your child will be engaged at camp in activities that involve risk. Camp Fowler complies to all NYSDOH risk management requirements. Your child's safety is our deep concern.

It is very important that you read the Release & Waiver form. If you have any questions regarding the information please do call the office  at 518-631-6789 and speak with the director regarding your concerns.

Your child can not attend camp without this form reviewed, completed and signed by a parent and/or guardian. The signed form needs to be handed in at arrival and check in.
